May I bill my insurance for my treatments with Dr Paradise?
Yes, Dr Paradise accepts Motor Vehicle Injury patients and work injuries, she is happy to bill your insurance for services rendered. Dr Paradise is OUT OF NETWORK for all insurance and is able to bill insurance if you have out of network benefits. She does not accept Medicare, Medicaid, or OHP.
How much will my appointment cost?
This depends on several factors, including the complexity of your case, and the number of services used during your treatment. Appointment fees also depend on whether you’re a new or established patient and whether you have insurance benefits that you can apply to help cover the cost of your visit. If you are experiencing financial challenges I can offer a sliding scale. Please call or email Dr Paradise for more specific information.
Can you tell me what my insurance benefits are?
It is the patient’s responsibility to check with their insurance carrier as to what their out-of-network benefits are. This includes checking whether a provider and/or service is covered under their plan, what their copay or coinsurance is, or what their deductible is. We have an easy to use form to help you check benefits here.
I’m an established patient and my insurance has changed. What do I need to do?
Before coming into your next appointment, you will need to check to make sure Dr Paradise is covered by your new insurance and your new insurance benefits. Please bring a copy of your updated insurance card to your next appointment and let us know that your insurance has changed. If you do not do this, you may be liable for the full cost of the appointment at the time of service.

Yes, I stock a full pharmacy of high quality supplements that are independent lab tested for purity and standardization. The supplements I choose are well researched and highly effective.
What do I need to bring to my new patient appointment?
For insurance patients, we require that you bring in a copy of your up-to-date insurance card. Please have all your new patient paperwork filled out before your appointment. I appreciate having copies of any recent lab work and diagnostic imaging reports (x rays, MRI, CAT scans, etc.)

What is your policy for late cancellations and missed appointments?
The clinic policy is at least 24 hours notice to cancel an appointment. Both late cancellations and missed appointments are subject to a fee. I work to honor your schedule and our time together, please respect my time too.
